Who Should Use This Form?
If you’re applying for a visa to enter New Zealand, maintaining an acceptable standard of health is crucial. The Chest X-ray Certificate (INZ 1096) is required to assess whether you meet these health standards. This form is necessary if you are:
- Applying for residence in New Zealand.
- Applying for a temporary entry visa intending to stay longer than 12 months, unless exempt (e.g., military, diplomatic visas).
- Applying for a temporary entry visa intending to stay between 6 to 12 months and have visited or are from a country with a high incidence of tuberculosis (TB).
Children under 11 years of age and pregnant women are typically not required to undergo a chest X-ray unless requested explicitly by Immigration New Zealand.
Submission Process
Most applicants can submit their health information electronically via a panel physician. To find out if this option is available, visit the Immigration New Zealand website. If electronic submission is impossible, the medical clinic must send the completed form directly to the Health Assessment Team in Auckland. You may be responsible for any courier fees involved.
Important Notes for Applicants
- Previous X-rays: If you submitted a chest X-ray certificate within 36 months with an earlier application, you might not need a new one, provided Immigration New Zealand has retained your last medical information.
- Special Cases: If you have spent six consecutive months in a high TB risk area since your last X-ray, you must submit a new certificate, regardless of previous submissions.
Completing the Chest X-ray Certificate
- Radiologist Examination: The chest X-ray must be completed by a certified radiologist. You may require a referral from a registered medical practitioner to undergo the X-ray.
- Identity Verification: The radiologist or radiographer must verify your identity by signing and dating your photograph attached to the form.
- Language Requirements: The certificate must be completed in English. A certified translation must be provided if any specialist reports are in another language.
After the Examination
Once the examination is complete, the radiologist will submit your chest X-ray certificate and any other required medical certificates to Immigration New Zealand. The submission must occur within three months of the examination date. Your application will be assessed and may be referred to a medical assessor. You may be required to undergo further tests or provide additional reports at your expense.
